Obtaining Form 1095-A

Did you or anyone in your household have health insurance through the Marketplace? If so, you’ll need Form 1095-A to file your individual tax return. You should not file your tax return without an accurate 1095-A form. This form provides details of the health insurance premiums paid and premium tax credits used.

The 1095-A is mailed out by mid-February however it may be available online by mid-January.

To obtain Form 1095-A online, you will need to follow the steps below:

  • Log into your HealthCare.gov account HERE
  • At the top of the screen, click “Go to My Applications & Coverage”
  • Under “Your existing applications,” select your application for the prior year.
  • “Tax Forms” on the far left
  • Download the 1095-A
